After a comprehensive examination, your doctor will analyze the diagnostic findings and develop a personalized treatment plan just for you. Before starting your treatment, we'll provide a clear breakdown of your treatment with an initial estimate—so you’ll always be informed about your expenses. We’ll also collaborate with your insurance provider to help you understand your benefits for dental and periodontal treatment, when applicable. Additionally, we’ll explore payment options to assist you in managing your remaining balance.
We are committed to providing you with the best possible care. Please understand that payment of your bill is considered a part of your treatment. We require that you read and sign the following financial policy prior to any treatment.
Payment is due at the time of service

Payment is due at the time of service, unless prior arrangements have been made. If you are insured, payment of any deductible and your estimated portion of your treatment are due at the time of service.

We accept cash, checks, MasterCard, VISA, Discover, and American Express.

An appointment cancelled less than 48 hours in advance, as well as missed appointments, will be charged $100.00. If your appointment is for a surgical procedure, you will be charged $100.00 if cancelled within one week of the appointment, and will be charged 50% of the total procedure if cancelled within 72 hours. Please help us serve you better by keeping your scheduled appointments.

INFORMATION FOR PATIENTS WITH INSURANCE

Your insurance is a contract between you and the insurance company. We are not a party to that contract. While filing insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date the services are rendered.

Our fees are covered up to the maximum allowance determined by each carrier. Any difference is the responsibility of the patient.

Not all services are a covered benefit in all insurance contracts. Any of these services are the responsibility of the patient.
We will gladly discuss any questions relating to your insurance benefits.
FINANCE CHARGES

A finance charge is imposed on any balance not paid in full within 60 days of the date services were rendered. The balance on which any finance charge is computed is determined by totaling the charges not paid within the required time period. The finance charge is set at a periodic rate of 1.50% per month(18% annual percentage rate), with a minimum finance charge of $.50.
